Registration Fees

$250 - Physicians

$175 - All other health care and clergy-based providers

$50 - Students, trainees and family caregivers

$450 - Group, three participants (This fee represents a savings off the equivalent individual registration fees.)

Participants will receive:

  • Eight-module curriculum
  • APPEAL DVD - professionally-produced DVD featuring African American perspectives on end-of-life care
  • Continuing education credits
